Power Wheelchair (PWC) or Scooter Evaluation
You can benefit from the power wheelchair (PWC) or scooter evaluation if you have:
- recently obtained a PWC or scooter
- an increased number of “incidents” to self, others or objects
- experienced a decline in physical or cognitive function due to injury or illness
- difficulty maintaining attention or concentration
- decreased reaction time, (such as inability to stop suddenly if another person were to cut in front of you while driving)
- decreased safety awareness
- vision impairments resulting in difficulty seeing signs, depth perception or distance between other cars
- signs of increased confusion
The evaluation includes:
- judgement
- ability to navigate through the environment
- decision making and problem solving
- driving skills
- physical and visual ability to operate the chair
- inside “on road” driving evaluation
- outside performance evaluation, if necessary
- adaptation and/or modification suggestions, if needed
The evaluation & follow-up process involve:
- The team is comprised of neuropsychologists, psychologists, and occupational therapists who provide on-site feedback at time of evaluation.
- Follow-up services are available with an on-site therapist and client if further therapy/training is recommended at time of initial evaluation.
- The OT is available to assist therapists or clients with the process of obtaining a powered mobility device, if qualified per Medicare with a medically necessary diagnosis.
Referrals are welcomed from healthcare professionals, facility staff, family, friends or self-referrals. A doctor’s referral is required for an initial OT assessment.
CGS accepts Medicare assignment and participates with many other insurance plans.
